CDS Jersey Order Form

The LHA is pleased to provide this CDS Jersey online order form for the 2026-27 season.

Please complete a jersey request for each player on your team noting their name, jersey size and jersey number.

For each additional player, please click "Add a Player".

If your team would like to order practice jerseys, please select the number of jerseys by colour and size. Please click "Add Another Colour or Size" until all the practice jerseys required have been ordered.
