CDS Jersey Order Form

The LHA is pleased to provide this CDS jersey order form for the 2026-27 season.

Please complete a jersey request for each player on your team noting their name, jersey number, and jersey size.

For each additional player, please click "Add Another Player".

If your team would like to order practice jerseys, please select the number of jerseys by size and colour(s). Please click "Add Another Size" until all the practice jerseys required have been ordered.
